HOW TO ORDER TISSUE FROM SOUTHERN EYE BANK
Welcome to our team! Southern Eye Bank prides itself on distributing ocular tissue to surgeons in need of a corneal tissue, scleral patches or education and research tissue. Because of the generosity of Louisianans, Southern Eye Bank is able to provide this Gift to those in need.
To order ocular tissue, the most effective way of doing so is by faxing a surgical request to 504-891-2401. Our team will reveiw each request and verify your order within 24 hours. You may also call our office at 504-891-3937. A copy of the Surgical Request Form may be printed from this tab.
Our office will need the following information in regards to your patient:
- Patient name, unique identifying number, sex, race, date of birth and address. Also state the performing surgeon, name of surgery center or hospital (please include address of surgical institution). Diagnosis and eye needing surgery is also required.
- Please inform us if the tissue needed is for pre-cut corneal tissue for use in Endothelial Keratoplasty, full-thickness cornea for use in Penetrating Keratoplasty or scleral shells.
- Surgery Centers / Hospitals will be invoiced within 7-10 days post receipt of tissue. For your convenience, we can e-mail invoices. for international and intranational surgeries.

If you have a patient who would like to share his / her story, please contact us with the information and we will share their story with future recipients who are anticipating their surgery and also with donor families who have graciously and selflessly donated their loved one’s ocular tissue.
